As a key member of our outpatient practice, you'll play a critical role in delivering high-quality, patient-centered care. You'll work closely with our medical director and care team to develop and implement innovative care strategies that drive exceptional patient outcomes.Responsibilities

• Provide comprehensive primary care to patients in an outpatient setting
• Partner with the care team to develop and implement personalized care plans
• Utilize our proprietary Electronic Health Record to track patient progress and optimize care delivery
• Collaborate with colleagues to share best practices and improve patient outcomesRequirements
  • At least 2 years of primary care experience required
  • Currently licensed or ability to obtain licensure in the state of Arizona
  • Excellent communication and interpersonal skills
